Schatz: We don’t know for sure, but the likely bombing of an elementary school with one of our Tomahawk missiles is not a one-off mistake. It is the result of a change in policy so that the United States has way looser rules of engagement — way more tolerance for civilian harm.

I’d say that most of the fraud in American healthcare is done by the insurance companies when they purport to provide cover and then refuse treatment or the general overcharging and over treatment by healthcare professionals. Fraud by patients is probably way below the preceding examples.
