In Duviri, where your Kaithe is stabled, there is a stable door behind, with a hidden opening on the right. Jump up and in.
In Hollvania, look for Gordon Freeman, and there's a PC there. Click on that, and there is also Minky's concept art there as well.
In Deimos, there is a pillar to your right on spawn. Jump up on top of that.
In Zarriman, head towards the mission lift, but walk in to the wall to the left of it. There is a hidden opening. Follow the path to the end.
Duviri and Hollvania next
In Cetus - Look at the balloon to your right, and walk towards and past it. You will walk into a wall. DE is on there.
In Fortuna. From start, jump up to the right, then jump up ahead of you, and it's on the barrier.
Deimos and Zariman next
I haven't seen Nosfera 1 yet, let alone Nosfera 2.
