In the report the Joint Commission states that these will become Joint Commission standards on January 1 of 2027 (a year from now).
In short, (SRE 5) *anything* that hasn't been prospectively identified and cleared for MRI safety (this includes people and objects or equipment) that crosses the threshold to enter the MRI scanner room qualifies as a serious reportable event.
SRE 5. Introduction of an unapproved, unscreened, or inappropriately approved device,
implant, or object into an MR Zone IV area, regardless of the outcome, and,
SRE 6. Patient harm associated with an MRI–related thermal injury
