Increased Medicaid eligibility checks: starting in 2027
New Medicaid work reporting requirements for individuals covered under expansion: states that have expanded Medicaid must implement these requirements by April, 2027, though they can seek federal approval to delay this until January 1st, 2029.
Restrictions on Medicaid for most legal resident immigrants (doesnβt apply to green card holders): 10/1/26
Reduced fed funds for SNAP for most states: Fed support for statesβ administrative costs reduced fall of 2026, & fed support for SNAP benefits decrease fall 2027 to fall 2029 at the latest.
1/1/2026, all lawfully present immigrant residents with incomes under 100% of the federal poverty level will be ineligible for ACA marketplace coverage. In 2027, DACA recipients, Temporary Protected Status, asylum seekers, & most others without a green card will be ineligible for ACA subsidies.
Increased ACA premiums: 1/1/2026.
Ban on Medicaid coverage for Planned Parenthood: starting in 2026 (note: temporarily paused this provision while a lawsuit challenging it proceeds)

Restrictions on Medicare eligibility for most immigrants other than green card holders: The Social Security Commissioner will review Medicare recipients to determine their coverage eligibility on a rolling basis over the next 18 months.
Expansion of SNAP work reporting requirements to older single adults without dependents and adults with dependents over age 14: As soon as the Department of Agriculture publishes rules or guidance to implement (could take several months at least)
Changes making enrollment in ACA health plans more difficult: these changes technically take effect immediately, but in practice will roll out over the next year as federal agencies issue rules and guidance. The open enrollment period will be shortened by one month starting this fall.
Many of the cuts or changes that remained will not immediately take effect, with some of the worst delayed for several years. Hereβs a rough timeline of when some of the most damaging provisions are expected to go into effect:
Trillions of dollars in cuts will be made to Medicaid, ACA, SNAP, and Medicare, and federal immigration agencies will gain tens of billions of dollars to expand their attacks on immigrant communities. Meanwhile, the tax cuts made or extended benefit the most wealthy individuals & corporations.
