On the plus side, in RapidILL/Alma resource sharing there's now a way to set a configuration for a user group or flagged user that any requested documents need to come to the requesting library for remediation first before going on to the user. Maybe you can convince illiad to add something similar?

It's such a huge problem for ILL! Our consortium found out a few years ago that even if the supplying library remediated the PDF, RapidILL was automatically adding a cover page that was breaking some of the accessibility features. (Canada so our laws around this are a bit different)
