Large Language Model Influence on Diagnostic Reasoning This randomized clinical trial evaluates the diagnostic performance of physicians with use of a large language model compared with conventional resources.

A friend sent this article to me today.

It feels like a good moment to remind us: the LLM outperformed physicians on STRUCTURED cases.

Someday the AIs may perform better than us in the unstructured, messy, real-world patient encounter...but that day is not today.πŸ©ΊπŸ›Ÿ

How to not be completely wrong about air filters A new RCT of HEPA filters for residential care homes gets heads spinning

Excellent analysis of what happens when a clinical trial is under powered. In this example, it's the use of air purifiers in nursing homes and preventing respiratory tract infections.

Results suggest it might work. Or it might not.

The preliminary sequence from the H5N1 human case in British Columbia has been posted and it is not good news. The virus potentially has a quasispecies at HA residue 226 (H3 numbering). This is bad news because we know that mutations at residue 226 can increase binding to human receptors. 1/

US health officials report 1st case of new form of mpox in a traveler Health officials say they have confirmed the first U.S. case of a new form of mpox that was first seen in eastern Congo.

Health officials in California have confirmed the first case of mpox clade I in the US. Patient is a returning traveller's who traveled to eastern Africa. This is not unexpected because the world is interconnected and there are ongoing outbreaks of clade I mpox in Africa.

We have found the opposite 2 be true during this πŸ‘Άoutbreak. Incorrectly obtained NP swabs (not reaching the OP) have been false (+) 2 begin w/ only to turn (+) when retested in the PICU. And kids w/ progression on empiric S.pneumo, GAS & Staph coverage have ultimately required Azithro + steroids.

Great point!

We often advocate for a unifying diagnosis (Occam’s razor), but in people with immunocompromise, it’s important not to disregard Hickam’s dictum; β€œPatients can have as many diseases as they damn well please!”
