Stigma is a major contributor to the SUD treatment gap.
We need urgent, evidence-based training for HCPs in non-stigmatizing communication and the integration of peers with lived experience into treatment teams.

We identified three key themes through Reflexive Thematic Analysis:
1️⃣ "Institutional Stigma" (Addiction viewed as a moral failing).
2️⃣ "Barriers to Care" (Symptoms dismissed as "just the addiction").
3️⃣ "Cost of Disclosure" (Experiencing hostility/contempt after revealing use).

Our mixed-methods study (N=119) confirms the "why-try" effect in healthcare: Internalized stigma was a significant predictor for all three care-limiting behaviors.
When patients internalize the message that they are "devalued," they withdraw from care to protect themselves
