Pleuritic type chest pain on deep inhalation in a profound/prolonged neutropenic patient w pulmonary nodules...you should be thinking about pulmonary IFI like aspergillus, esp if not on anti mold triazole or sub-therapeutic. #idsky #transplantID #oncID

#transplantID pearl: belatacept conversion from CNI - look out for cmv reactivation with high grade viremia w end organ disease and sometimes longer time to viral clearance. Also: bk viremia w graft issues or pcp (but everyone is on bactrim so more rare to see) #idsky
