there is already OCD as a possible disorder in mewgenics, so here is my idea for a scrupulosity specific version:
"scrupulosity: whenever you down a unit, gain stacks of fear and confusion equivalent to the amount of units killed that turn. debuffs cured when hit by a move that does holy damage"
