Academic clinicians, Iβm preparing a report on why professionals need more than 2 years training to be competent and safe. Iβd welcome suggestions of articles on the complexity of clinical knowledge and how it is acquired. Thanks if you can recommend any (I have my own obvs). #medsky #academicsky
Trial high dose nasal corticosteroids for 2 weeks with saline rinses.
Ear exam is normal -Iβll refer you for an audiogram.
How much nonsense did that just save?
Also I can think of better use for an ENT regβs time than holding MDMs with various members of the primary care team to decide whether to make an ENT referral in the first place. Training is short and thereβs a lot to cover!
And now for the parody that makes a mockery of the real NHS England pathway
The Green party have 4 MPs and have made HUGE GE and council gains.
Yet all we see in the media is about the far-right circus that is Reform UK.
The media are gagging for a far-right victory and it's sickening.
Me too. Wikipedia is the internet's single most valuable contribution to human knowledge. It's clearly galling for Musk that he can't turn it into another billionaire-owned misinformation machine.
when you feel even the smallest spark of "i want to make something" or "i want to do something", do whatever it takes to hold onto that feeling, because it's so easy for your dreams to get swallowed up in the everyday struggle to survive
The cards are on the table.
After years of suppressing discussion, ignoring concerns & saying not GMC/NHSE remit to set scope, GMC donβt miss a beat immediately advocating for less defined scope when someone else does it.
GMC continue to do a disservice to everyone involved, including AAs.
βBut its support of NHS, GMC and government plans for PAs placed (RCP) in direct opposition to its members.
This was a sobering reminder to membership organisations that steer their hubristic agendas away from their members.β
Kamran Abbasi, BMJ Ed-in-Chief
The GMC attempts to subvert & overturn the defined Scope set for PAs & AAs by Royal Colleges of Anaesthetists and RC of Physicians after previously refusing to set any ceiling of practise for them and redirecting the responsibility to the RCs & Specialism Societys. Now it attacks the Scope they set!
According to Reform UK's election manifesto, the UK is "ruled by an out-of-touch political class who have turned their backs on our country". So it's heartening that one of the party's latest big donations comes from a patriotic British businesswoman who lives in, er, Monaco. On 5 December, the Electoral Commission released donation figures covering the third quarter of 2024. Hepburn Bio Care UK Ltd, a small company based in Southampton, gave Reform Β£20,000 β its third largest donation that quarter. Hepburn Bio Care is owned by founder Margaret Hepburn, a Brit who has lived in the tax haven since at least 2005. The firm makes "environmentally friendly" cleaning products for cruise liners, including major operators like Carnival, and describes itself as a "multimillion dollar company". However, the UK branch which donated to Reform is listed as a "Small Company" that only publishes "filleted abridged" accounts and shows a small annual profit of Β£12,000 in the latest (2023) accounts β less than the sum it gave Reform. Hepburn Bio Care does have branches in Monaco, Dubai and Florida, so it is possible the multimillions of dollars are in their accounts. It also supplies products to the "superyacht industry", suggesting Reform is not just a party for the have-nots, but the have-yachts too. The article appears to be highlighting what it sees as irony in a political party that claims to represent people against an "out-of-touch political class" receiving significant donations from a Monaco-based businesswoman whose company provides services to luxury industries like cruise ships and superyachts.
Blimey, this is big. Reform UK is already effectively receiving foreign donations. Via Private Eye
Nice story this.
8 kids from Gaza with complex medical needs arrived in Ireland last night for treatment.
A total of 30 are expected to arrive for medical care in Ireland on the scheme organised by the Depts of Health and Foreign Affairs and the Irish Red Cross

Also, I thoroughly recommend starter packs for finding interesting NHS / health / social justice people. Here's an example of one... go.bsky.app/GZJeMY5

Hopeful noises coming out of GP/Govt contract negotiations - but there's a lot more still to do. Glad continuity of care is finally a buzzword (buzzphrase?)
Some concerns that all the money so far announced might be swallowed up by β¬οΈcosts (NICs, NMW etc). Huge gratitude to KBS @the-bma.bsky.social
Story time:
We had a patient admitted to the PICU admitted with recurrent fevers despite multiple broad spectrum IV antibiotics. They could not communicate well at baseline and their blood cultures were negative. After a few days we decided to consult ID.

It's that time of year where I post this one
Absolute highlight of 2024 Congress for me β¨
Please give it a listen if you missed it in person!
We (both men and women!) need to be discussing this more to make sure there are less barriers to the best people being where they need to be in order to effect change π£οΈ
More complex, more elderly, more need for preventative care
More GPs needed
But there has been a 20% cut in funding
GPs / Social care / 3rd sector has been hit by NI employers tax rise & staff costs
Canβt think of anyone better than @suhana.bsky.social to offer guidance about the opportunities and challenges for women leaders in medicine. π
Not yet. Nor shevelled at the moment. But you will probably know about the Recombobulation lounge at Mitchell Airport in Milwaukee..
What a shock. Canβt expect GMC to dip into that funding for their own private healthcare though (which tbf is also taxpayersβ money) π
The issue is the motivation & incentives in the NHS run in direct opposition to patient safety.
Doctors are routinely asked & expected to practice unsafely. The unsafe scope & unsafe supervision of PAs is part of that problem.
PAs don't make financial sense w/ the supervision model needed.
Just a reminder:
All vaccines used in public health have been rigorously tested. Their benefits greatly outweigh their risks. While a few people have a medical reason for not taking vaccines, most people (and the population overall) will benefit. Doctors who claim otherwise are irresponsible.
