I would try a straight ring instead of curved, that will close the gap a bit. If you dont want to have to get new parts, you could also consider trying snug fitting straps/harness. That would eliminate a lot of slippage in your current fit.

The size of the cage is irrelevant for this matter. Here's a diagram to better visualize it.

It happens, I went through a few cages where that would happen often.

The underside. The distance between the base ring and the proximal end of the cage. That space is where the slippage occurs.

Sounds like the ball gap on your device is too big for you. You'll need a different size to prevent this. It should be impossible for a testicle to pass through.
