Really useful paper. Traditional dosing strategies (200mg IM loading, followed by 800-1200mg/day) don't reflect its half-life. Typical loading doses of around 600mg may not need any follow-on regular dose (or a very-much-less than 800mg/day dose if used)

I agree, but sometimes that comes at a cost to reducing access. Basic E.g can we provide the excellent care to one person, or can we provide good care to two people and free space in the system for someone else to get out of A&E onto amu and therefore an ambulance to offload their patient.

Iโ€™m just not a big fan of the outsourcing model. I work in an โ€œnhs hospiceโ€, youโ€™ll be familiar with a similar model in Oxford. Weโ€™re efficient, we see more patients than the hospices around us. Think the responsibilities and integration with the local system that come with the funding will be key.
