Yi-Hsin Hung, MD
2025 Mentee Award Winner
Cardiology Fellow
National Taiwan University Hospital
Taiwan
Two weeks ago, I had the incredible opportunity to be in Italy to meet my mentor, Dr. Carigi.
Never could I have imagined such a moment - walking through the midnight streets of Italy, sharing gelato and stories about life, careers, and, of course, cardiac amyloidosis.
Our project discussions flowed naturally, grounded in a shared vision and quiet determination. I'm deeply grateful to Dr. Carigi - not only for her expertise, but also for her warmth, encouragement, and belief in each of us. Being welcomed into that space of trust and generosity is something I'll carry with me!

Key Question
Dual antiplatelet therapy (DAPT) de-escalation improves outcomes after percutaneous coronary intervention compared to standard DAPT. What is the impact of sex on the safety and efficacy of these strategies?
Key Finding
In this pairwise and network meta-analysis with prespecified subgroup interaction, DAPT discontinuation reduced major adverse cardiovascular events compared to standard DAPT in females but not in males, while reducing major bleeding in males but not females. Conversely, no sex-related differences were found for P2Y13, inhibitor switch or dose reduction.
Take Home Message
Compared to standard DAPT, DAPT discontinuation may be particularly effective in females while P2Y12 Inhibitor switch or dose reduction may perform best in males.
[Graphic showing the best strategy for females and males to treat major bleeding and MACE, with a graph of the S-Node rankings of each treatment for major bleeding, MACE, all-cause deaths, NACE, CVD, MI, stroke, & any bleeding]
Sex matters in DAPT de-escalation after PCI.
In 71,000+ patients, aspirin discontinuation reduced major CV events in women but not men, while lowering bleeding in men but not women. Precision care requires sex-specific evidence.

There's an urgent need to uncover the distinct mechanisms driving hypertension and its cardiovascular complications in females - an underexplored population. Closing this knowledge gap is essential for developing more effective, sex-specific treatments. - Dr. Chengxue Helena Qin, Monash University, Australia
A preclinical study identifies a key link between estrogen and ANXA1, a protein that protects against hypertension-related heart damage.
Lower ANXA1 led to greater CV damage in female models, reinforcing the need for sex-specific research and care.

"Stroke is a major cause of death and disability that may be preceded by distinct risk factors in women vs. men. Understanding earlier life, sex-specific stroke risk factors can help identify high-risk individuals who can be targeted for preventive intervention." - Dr. Abbi Lane, University of Michigan
Research shows pregnancy complications like preeclampsia or gestational diabetes can signal future CV risk.
We believe that supporting women physicians helps ensure these early warnings lead to tailored prevention & better long-term heart & brain health.
