I’m playing with the idea that maybe Josh had always been sickly, which made Alex want to be a doctor and his parents used Alex for spare parts to keep Josh alive?? And maybe instead of the lake incident, Alex made a conscious decision to not give Josh his kidney or smth and that’s what killed him?

He’d definitely be responsible for his brother, tho I’m playing with the idea that Josh was always sickly, which made Alex want to be a doctor and his parents essentially used Alex for spare parts for their chosen son since they knew he was going to be sacrificed anyways??
