It means the person fostering adopts them instead of letting them be adopted into another home! It's not really failing but successful foster is supposed to be only a temporary home:)

This is Magda! The person she was with was threatening to abandon her and her sister out in the country so she ended up at the rescue I was fostering for. Formerly Miss Tutu, Mags was my first foster fail 2 years ago. She is 10 and now living her best life as my studio assistant!
