I think the thing which keeps me at least a little angry at all time is knowing that the premise of scarcity used to justify all this gatekeeping is the result of insidious failures to keep the ultra rich from sucking all the cash and resources out of the public sphere over year and years.

A misconception is docs have some kind of inherent “truth sense” and are appropriate gatekeepers for restricting/scrutinizing service and income provision. For conditions like addictions and mental illness though, our primary way gauging level of disability is from what our patients tell us.

So much of medical care (especially for marginalized/oppressed populations) in late capitalism centres around reinterpreting what our patients tell us and then reconfiguring this information into institutionally legible forms… because our patients inherently are not trusted.
