On occasion, I've used the "Summarize notes" AI function of #EPIC to see how it summarized the medical record. Every time, there were *major* factual errors in the #AI output. Garbage in --> garbage out. A human can sort out the crap. AI sanitizes and launders it, which is dangerous!

There are different mechanisms and disease conditions that create injury to the AV node versus the His Purkinje system. There are actually not too many things I can think of that impact both of them, other than perhaps aggressive catheter ablation in that area!

I don't have a good explanation for you.
It's possible to have His bundle disease with QRS complexes that demonstrate LBBB, RBBB, narrow QRS, but the very long PR intervals and obvious Wenckebach with narrow QRS would be very unusual for His Purkinje disease.

I've seen a tech or nurse get an EKG on a patient and inadvertently forget to charge the name to the new patient. Thus, wrong name on EKG and it goes in the wrong chart and there's no way to detect the error (other than seeing an anomalous looking EKG in a patient's chart).

Very strange. Different QRS, different PR interval observations. Would be very unusual for a person to have both AV node and His-Purkinje block in precisely the same timeline. πŸ€·β€β™‚οΈ
If that asystole is indeed the same patient, so glad the pacemaker is in!!

Very likely the first EKG is the wrong patient and it was mislabeled. Almost certainly different patients with different AV conduction patterns and different QRS complexes.
Ekg #1 is narrow QRS, Mobitz 1, AV node level block. Benign.
Ekg #2 is LBBB, normal PR, then paroxysmal AV block. Ppm ASAP!
