For me it's the missing thumb on the top hand and it's got that weird, plastic-look to me. The lighting and shadows also look off, and it seems very 'clean' for an old photo. Could be a real photo that was 'cleaned' with AI, I guess. I hate that I immediately doubt stuff like this now. D:

The game does /slightly/ draw attention to this by making the portrait borders kinda wrap around one another in the turn order.
What I'd say which might not be immediately obvious is, in combat if two characters are listed to go one after the other you can click their portraits in the turn order to switch between them and you can just do their actions in any order you like, which can be useful for setting up combos.
I've only ever heard the phase "the world has lost all meaning" in that context, but I know exactly what you mean. Sometimes trying to read the word in that state makes it look completely alien.
