Yes. This is a game changer. Beyond being able to replace AVS, it allows one to β€œsee” the underlying pathological contributors to a large proportion of β€œidiopathic” hypertension. This is another piece of evidence that should make clinicians rethink the pathogenesis and treatment of hypertension

Podcast Episode: Primary Aldosteronism | HBP Guideline | JACC JACC’s November 4 issue features the High Blood Pressure Guideline surrounded by a series of expert commentaries. In this video interview, JACC Deputy Editor Erica S. Spatz, MD, FACC, speaks with author Anand Vaidya, MD MMSc. Listen in for an insider view of the evolving understanding of primary aldosteronism in light of the 2025 high blood pressure guidelines. Dr. Vaidya emphasizes that primary aldosteronism may be a common, modifiable contributor to hypertensionβ€”potentially affecting up to 25% of patientsβ€”and advocates for simplified diagnostic approaches and broader use of mineralocorticoid receptor antagonists to improve outcomes.

Beautiful study. Stop the madness.
Low-renin hypertension is by far and away some manifestation of primary aldosteronism pathophysiology. Even if you get caught up in names and definitions and can't accept this, it responds beautifully to MR antagonist therapy, so just do the right thing.
