$5 million boost for world-first trial tackling strep infections in both children and adults - Doherty Institute A major new clinical trial, the Streptococcal Adaptive Platform (STRAP), has received $5 million in funding from the Australian Government’s Medical Research Future Fund (MRFF) to improve treatment for life-threatening streptococcal infections across all age groups. Lead by researchers and clinicians from the Doherty Institute, Murdoch Children's Research Institute (MCRI), and other national and international partners.

Every retrospective IVIG publication ends the same. Your chance to change that conclusion coming soon… STRAP in, STRAP is coming!

Clindamycin Plus Vancomycin Versus Linezolid for Treatment of Necrotizing Soft Tissue Infection In this retrospective, quasi-experimental study, there was no difference in 30-day mortality in patients receiving treatment with clindamycin plus vancomyc

So… linezolid or clindamycin? 😂

Hopefully @erinmccreary.bsky.social is on the case!

0 reason to think things would be difft in non HIV, children, xenomorphs, non carbon based life forms, or any other excuses stubborn deniers have. My suggestion? Do some yoga. Meditate. Light incense. Relax. And stop torturing your patients with 2 weeks of amphoterrible. Subtle as always, I know.

The reliance on high-dose fluconazole and flucytosine as the basis of induction therapy in the AMBITION-cm study might not be
pragmatic in high-income settings, where more comorbidities occur, potential drug–drug interactions need to be carefully considered, and the risk of hepatotoxicity is less tolerated than in low-income settings. In the USA, only a third of patients completed
the 14 days of flucytosine. Although some experts support the inclusion of the AMBITION-cm triple regimen as a primary option in high-income settings, other experts call for further comparative trials in high-
income settings to assess the regimen’s effect in patients with HIV and patients without HIV (in whom no supporting data exist).

Nice editorial & study!

Are there any good reasons to think different results would be seen in HIV-unrelated cases? Kids?

Or are we still in the same place as in the Lancet ID 2024 guideline? ‘Some experts’ vs ‘other experts’?

As cases of a rare, deadly infection rise, doctors worry fewer teens will get vaccinated Under Robert F. Kennedy Jr.’s guidance, the CDC no longer recommends routine vaccination to protect against meningococcal disease.

From first hand clinical experience, meningococcal sepsis and meningitis are indelibly etched in my mind—diseases that can progress rapidly from flu-like symptoms to loss of life or limb—even in previously health children & young adults

Vaccination had reduced rates up to 90% – don't give up now

How can we accelerate human trials of vaccines, therapeutics and diagnostics against priority pathogens in advance of epidemics and pandemics? Well, if we can identify safe, weakened versions controlled human infection model trials might be possible:

Progress in vaccine development only happens when people are willing to step up. Challenge trials are one of the most powerful tools we have for speeding lifesaving vaccines. If we want faster, more equitable global health outcomes, we need to expand and support this research model.

✓ Act prophylactically at low pathogen burden

Some NICUs don’t specifically cover anaerobes for NEC, although more to do with understanding of the developing GI microbiome. Your Q isn’t whether anaerobes are there, more what happens if we don’t go out of our way to treat them? There are some cholangitis & asp pneumonia studies 🤷‍♂️

🤔 could consider when clindamycin was abandoned for IAI (because rising Bacteroides-R). If this was preceded by ⬆️ refractory cases that would be convincing-ish. And how about before any of these drugs were available? Anaerobic culture standards probably haven’t been consistent enough to be useful.
