Agreed
Xennial here. We're just having a festival of staph over here. Luckily they're all helping the SNAP trial answer a few outstanding questions on how to manage.

I'll have a think.
- beware the septic patient with an initial normal wcc.
Also love this list and the approach.
- find out why the micro was taken. You'll spend as much time explaining what isn't important as what is
- a fever with low platelets, normal wcc and mild raised transaminases is doxycycline deficient until proven otherwise
Getting decent micro.
A cute drawing of an iguana and some zoonoses associated with them
Everything is a zoonosis! Drawing/listing challenge.
#29 - Iguana
Requested by @catherineberry.bsky.social and inspired by @iuidfellowship.bsky.social
Found an interesting report of Burkholderia pseudommallei abscess in a pet iguana, could that have zoonotic potential?

Amazing. Thanks so much.
Ok since I looked this up. Good overview on BC contamination reduction pubmed.ncbi.nlm.nih.gov/31666280/
The Self study I quoted above looks to be the best of its genre.
Some authors are proposing 1.5 to 2 % benchmarks (Halstead et al.). We work on less than 3% currently.
So this is now the question in the genexpert age, what contamination rate are we aiming for and what is the acceptable stewardship and financial cost? And what interventions give the best bang for buck.
So great to see data in this population. I'm not going to lie though, I had to google flomoxef.

This is useful and provided by my micro colleague. Will look for more data. pubmed.ncbi.nlm.nih.gov/22476282/
Yes this is my question. What I do know is 3 stabs are unlikely to be acceptable to ED and probably not fair to patients either.
Yeah we're chatting about implementing single-site collection but would be contingent on not sampling from a cannula. So still two stabs for emergency patients.
Just published 🙂 pubmed.ncbi.nlm.nih.gov/41483490/
