A welcome instance of hope Evidence from a small island in Indonesia points to the successful elimination of a parasitic worm that causes a devastating tropical disease.

I often use antifungals for the surrounding infiltrates, and it's important. But the ball itself needs surgery.

I would guess this is not a failure. Just natural progression.

Likely to be related to source control and it is also a reasonable timeline.

Is it culture positive 5 months later? Or what makes it refractory?

In general, I never found mixed antifungals to be helpful.

This was my experience too. There is a paper out there, where I analyzed the data, wrote the methods, and much of the results, setting the tone for what the story of the paper will be. I then went to another institution, and the paper was published without my knowledge, and without my name.

This is such an interesting study. Many times in medicine we feel that something should be a risk factor for death. But it turns out not to be. And often, even when that data is clearly presented, people still continue to not believe it. Negative studies teach us often more than a positive study.
