So why not a different approach that's silo agnostic & vendor neutral? An approach that's more, flexible & intelligent? One where primary & secondary care share the same data repositories but in different ways, with real access control across all geographies
Agree, provided that it is the clinical record - interventions, obs, diagnosis, plans, examinations but needs to be vendor neutral, perhaps curated by clinicians, and reach across all boundaries where the above occurs. Access to such records could be managed in a far more intelligent way too.
The NHS electronic prescribing systems I saw or used were riddled with easy to achieve errors.
Very unusual Meds should blocked from prescription by Doctors below Consultant. Doses above ordinary Maximum again should require Consultant ID and password.

Strong argument for a single NHS Hospital Clinical 'Record' - and one that is accessible in England Scotland Wales and Northern Ireland.
What a catalogue of missed interpretation of blood tests results, contacting the wrong teams in the wrong hospitals, failure to send comprehensible messages, Ward Rounds without Checklists etc. A typical NHS hotchpotch which happens daily and sometimes are fatal. @ShaunLintern @@JKMVidimo
