Looks like I got it way off... I went back and found quite a few postcards from over the years with different prices. The one I quoted seems to have been from the 1970's (based on the stamp used). Here is one for the early 50's a little more in line with what we would have expected.
According to a postcard from the same period you had to meet the night's minimum cover in food and/or beverages. Weeknights $8, Saturdays $9...

D1) again, this is difficult to pull off at longer lengths. But you don't get good at a thing by not doing it.
E) YOU DON'T GET GOOD AT A THING BY NOT DOING IT done is better than perfect. Don't worry so much about "practicing right" just practice. You can adjust as you need as you go on.
