In a case like the R-word, the clinical usage is much more recent and widespread, and there are still people who would call a developmentally disabled classmate that. At some future point though, that term too will cease to function as a slur and subside into mere insult.

Itβs a fair question. I think a lot of it is language drift, yeah, and also context. e.g. Iβm aware that idiot and moron used to be clinical terms, though not the numbers, but nobody uses them that way any more. And lame is insulting applied to an idea, but not to a horse.
