Does this happen in all cases of a book breaking out? No. But I would wager that a majority of the books that do "break out" are ones with substantial marketing budgets and a strong team behind it pushing the book.

I just want authors to remember NOT to self reject.
Your book doesn’t have to be a perfect match to a MSWL
If the agent takes fantasy and you have fantasy take your shot! ❀️
