Two years later, I got a hysteroscopy ablation (plus anesthesia, which is not usually the case but I had extenuating circumstances). It's not covered by my insurance unless the gyno records a person as having heavy periods. Insurance was billed $6K, and I was charged zero.
per the ACA, sterilization is supposed to be covered 100%. I reviewed my records for 2021, and my insurance was charged about $36K. It was all covered except for $118, and I can't figure out why. Call your insurer and ask for the tubal billing code and cost.
