Check lists increase the completeness of documentation in clinical notes... question remains about impact on correctness of documentation. I see daily use of pro forma notes that carry forward the same incorrect information. Stopping to think before writing remains paramount.
Always more work to do on this... it seems some "pipelines" are more efficient. We have talked about this for >20 yrs in Aus and seen slow progress overall. There are some wins... our unit is 50% female, and 5 out of last 6 HPB fellows have been female.
