A normal O₂ sat in a child in respiratory distress ≠ they’re okay.
A low O₂ sat in distress is often a late and concerning finding
Conversely, a brief sat of ~89% in a well-appearing infant with bronchiolitis may be acceptable. Context matters

Neonatal listeriosis treatment:
➡️ Start IV ampicillin + gentamicin
➡️ Treat parenterally only
➡️ Once clinically improved and CSF is sterile, stop gentamicin
➡️ Complete 14–21 days of ampicillin monotherapy

Given maternal ingestion of raw milk + fever, vomiting, diarrhea, and preterm delivery, you should be concerned for listeriosis. 🚩
That said, other infectious etiologies must always be considered in both mother and newborn.

In a 3-center study of 738 infants started on HFNC in the ED, tachypnea + hypoxemia predicted escalation within 24h (AUC 0.75).

Rectal and esophageal probes both have their place. Consider the clinical scenario.
🔹 Rectal: widely available, easy to place—but lags during rewarming
🔹 Esophageal: best real-time core temp (intubated pts), reflects cardiac temp
In critical cases → consider 2 central sites for accuracy.
