The commitments require further concrete action to bring real change, and there are also reasons for caution. Housing is the ultimate foundation to good health and the strategy overlooks key proven measures that would help end homelessness.

We welcome ambitions set out in the Plan:

✅Improving existing funding streams to support specialist intermediate care.

✅Embedding housing officers in hospital discharge teams.

✅Improving access to care and challenging the stigma people facing homelessness experience.

Today, the Government has published its long-awaited National Plan to End Homelessness.

We’re delighted to see a firm commitment to ending the unacceptable practice of discharging people from hospital to the street. @pathwayuk.bsky.social has long called for this vital and overdue change.

Doctors are the experts on health, and what helps and harms our health. It’s therefore so powerful to see them standing up for the dangers faced by people experiencing homelessness and calling for action in the Government’s forthcoming homelessness strategy.

We support this call.

🤕 Better understanding of the needs of patients facing homelessness

We warmly welcome the news that, for the first time, housing status has been included in @nhsengland.bsky.social's Statement of Information on Health Inequalities, setting out what information Trusts and Integrated Care Boards should collect to meet their equalities duties.

✅ Freed up 13,000+ bed days for other patients by reducing readmissions

@pathwayuk.bsky.social’s hospital homelessness teams must be part of the solution for the cash-strapped NHS, and a central plank of the Government’s drive to end homelessness.

In 2024/25, the specialist homelessness teams:

✅ Helped 4,778 patients experiencing homelessness, a 41% increase on 23/24
✅ Reduced returns to rough sleeping by 62% and to sofa surfing by 33%
✅ Registered 70% of patients with no prior access to primary care with accessible GPs

With funding re-established, we’re thrilled that HIT’s life changing work is continuing.

Prioritising safe hospital discharge, the multidisciplinary team's highly skilled, person-centred approach has reduced the number of patients returning to rough sleeping by 60%.

Last week we had an excellent 2 days delivering inclusion health induction training for the brilliant St George's University Hospital NHS Foundation Trust's Homelessness Inclusion Team (HIT), one of the specialist homelessness hospital teams we support on our Pathway Partnership Programme.
