I'm willing to bet that, given how the campaign is running - an airpower, stand-off fires event to attempt strategic effect - that SecDef/POTUS were warned Hormuz would be in play, and that defense would be highly difficult to impossible, and those warnings were ignored.
Recall Iraq. Shinseki had a plan for a correctly-sized force to deal with the aftermath. Nobody liked that cost or the political message it sent. The administration chose a different plan.
And the corollary to this is: the plan for Hormuz was, don't start nothing, won't be nothing. Because all of the ways to fight it open are terrible. That's why JCPOA was such a boon to the US.
It's almost never a planning failure, always a choices failure.
There's a COA to keep Hormuz open: it costs a shedload in money, ships, personnel, and escalation risk. It isn't, oops we forgot the coats!
