Belangrijk dat mensen die getroffen zijn door andere post-acute infectieuze syndromen (#PAIS) waaronder zeker ook Q-koortsvermoeidheidssyndroom (#QVS) kunnen aansluiten bij de ontwikkelingen voor post-COVID.
Krachtige oproep voor betere zorg, uitzicht op goede behandeling en meer onderzoek naar Q-koorts. Ook belang van oprichten expertisecentra genoemd. Goed om te weten dat het Q-koortsexpertisecentrum in @Radboudumc al zo’n 15 jaar zorg biedt. Ook veel onderzoek toen daar nog wél subsidie voor was.
Vandaag #Qkoorts symposium in Den Bosch met o.a. Q-uestion, Q-support, de Commissaris van de Koning, de nationale ombudsman, nieuwe Q-koortsambassadeur en veel patiënten op weg naar erkenning en verbetering kwaliteit van leven voor iedereen die door Q-koorts is geraakt.

Er is wereldwijd verspreiding van een nieuwe omikron subvariant maar geen aanwijzingen dat mensen hier ernstiger ziek van worden dan eerder. Natuurlijk is er nog steeds kans op post-COVID en blijven de adviezen bij luchtweginfecties belangrijk:

We zien inderdaad de laatste weken iets meer opnames door corona maar nog steeds weinig. Ook landelijk was er alweer een daling van virusdeeltjes in riool.
In @radboudumc.bsky.social behandelen we in ons Q-koortsexpertisecentrum sinds 2007 mensen met ernstige of gecompliceerde vormen van acute Q-koorts en mensen met chronische Q-koorts. Ook zien we patiënten bij wie er sprake lijkt van Q-koortsvermoeidheidssyndroom.

Complimenten voor de snelheid waarmee deze Q-koortsuitbraak onder bosrendieren in GaiaZOO ontdekt is, zeker bij een diersoort waarbij Q-koorts nooit eerder is beschreven.

Ja klopt ME/CVS is ook genoemd in het stuk pathogenese en samenwerking op gebied van zorg en onderzoek. Was een goede sessie die hopelijk veel van mijn collega internisten echt bereikt heeft.
Take home messages:
- integratie van zorg en onderzoek biedt perspectief (#Lyme, #Q-koorts, #post-COVID)
- erkenning en samenwerking tussen patiënten, clinici en onderzoekers essentieel

Plenaire sessie op laatste dag #Internistendagen over #PAIS (post-acute infectiesyndromen) door Joost Wieringa met aandacht voor post-COVID, Q-koortsvermoeidhedssyndroom, post-sepsissyndroom en persisterende klachten na behandeling voor Lyme.
En @middeldorps.bsky.social zal trots zijn: nu bij de sessie over cardiovasculair risicomanagement op de #Internistendagen om als internist-infectioloog breder op de hoogte blijven in de interne geneeskunde.

Adviezen:
- bespreek de fout snel met de patiënt en familie
- deel het met collega’s en neem tijd om eigen emoties te verwerken
- leg alles vast, mag beknopt
Daarna mogelijkheden om van fouten te leren besproken met voorbeelden uit de praktijk.
Na het Europese infectieziektencongres meteen door naar de #Internistendagen in Maastricht. Na interessante presentaties over o.a. calcium/vitamine D en vaccinaties nu indrukwekkende sessie “(F)out in the open” over medische fouten, de gevolgen daarvan en hoe daarmee om te gaan.

Risk of avian influenza for general public at the moment still low, risk to people with occupational risk moderate.
Adaptive mutations may facilitate transmission between mammals including humans. This should be monitored closely.
Conclusion: we are poorly prepared for a new influenza pandemic!
Ferret studies:
- H5N1 viruses from birds and cows not transmissible from ferret to ferret
- H5N1 viruses from humans or other mammals can be transmitted between ferrets
Virus shedded by ferrets infected by H5N1 from a human in the air for several days.
Next presentation by Ron Fouchier “Recent changes in epidemiology of avian influenza in relation to human health risks”. Many unreported human cases, not detected (mild disease) so case fatility rate unknown.
In USA 1 death in 70 reported human cases. At this moment no human-to-human transmission.
Also spread to wildlife who can be both victim and vector.
Now 74 species of wild animals and 21 species of domestic animals affected
Recommendations:
- expand goals to include protection of wildlife health
- context-specific cap on poultry production: recommended 20 yrs ago but not implemented
Spillover to dairy cows in the USA and spread by virus contamination of milking unit and among farms by transport of infected cattle. Limited to mastitis in cows but high viral load in milk. Until now 41 cases in dairy farm workers, 24 in poultry farm workers and 5 other/unknown source.
Last session today at #ESCMIDGlobal 2025 on #AvianInfluenza. First presentation by @thijskuiken.bsky.social “Threat of highly pathogenic avian influenza to non-traditional hosts, including humans”. Important role of increasing poultry industry.
The second excellence award lecture at #ESCMIDGlobal 2025 is given by Arturo Casadevall: “Can global warming cause new infectious diseases?” He already answered before going to the second slide that this will be the case: “both the host and the microbe get in trouble by global warming”.

Also from @radboudumc.bsky.social IPC expert Corianne Meijer is now presenting about “High level of measles immunity in healthcare workers with unknown immune status”. Targeted vaccination after performing serology saves vaccines and we even might consider people born before 1970 as immune.
Second talk in this session by Annelies Mondelaers from @uantwerpen.be on “Preparing first-line health care workers for virus X”. Proud to participate in this important project from @radboudumc.bsky.social identifying knowledge and competency gaps and finding a positive attitude toward training.
