if the nyt obit is to be trusted, mcdonald's (commie ass!) parents named him after country joe stalin, which is where his nickname derived, & melton pulled the fish/mao connection after that.
and since it took me an embarrassingly long time to figure out: "the fish" doesn't refer to country joe's backing band, it refers to barry melton.
