I'm no expert, but the letters O, A and especially R look different.
Used to happen across all public hospitals in New Zealand (using three-letter, four-digit unique identifier) and Scotland (using ten-digit unique identifier).
I haven't worked in either of those places for many years so can't say what's happening now.
