ah, I see. It's possible she was phished with an oauth flow. Microsoft-owned oauth apps like Outlook are "pre-consented" so there's no warning as you log in, and also no app to revoke, you just wait 14 days for the stolen refresh token to expire I think? awful circumstances.
red team dev here. this sounds like they are on her device. If she is signed into her Microsoft account on Windows, the CloudAP auth provider is a local RPC to fetch Oauth bearer tokens for Outlook/M365. An attacker can automate the behavior you described, and it will survive password resets/2fa
