Outcomes of Infants Born at 21 Weeks’ Gestational Age This case series presents survival and morbidity outcomes for infants born alive and resuscitated at 21 weeks’ gestational age at a center that attempts resuscitation in selected cases.

So proud to be at @uiowaalumni.bsky.social where we just reported 6 neonates born at 21 weeks have survived to discharge with good respiratory and neurological outcomes thus far. We are continuing to try to advance care of infants when we can! #neotwitter

Caring for the most extremely immature infants There have been multiple publications concerning this issue recently, many from the tiny baby collaborative. The first 2 publications are about the overall approach to providing intensive care at e…

Love this quote "we owe it to the families to try with a dedicated team who believes in these babies". Survival at 22 weeks is not a myth! And I've seen that you can do it without morphine and phenobarb to improve neurodevelopmental outcomes! #Neotwitter

I agree this is a great article about changing the expectations in medicine to decrease burnout! Also talks about reducing service time for maternity and medical! So important to get everyone on the same page!

Young Investigator: Lindsey Knake, MD Harnessing AI to help the most fragile newborns

Proud of this young investigator spotlight! Lots of work to do!

β€œHow do we implement AI so clinicians trust these algorithms and can use them to augmentβ€”but not completely replaceβ€”their decision-making? That’s where I foresee the next frontier of AI research.”

Evaluating decision regret after extremely preterm birth - PubMed NCT04074525.

Loved the @nicupodcast.bsky.social episode on decision regret in ELBW. We had a 21 weeker that after 3 counseling sessions with family they decided to resuscitate. Baby didn't survive but they cherished the moments they had. Shared decision making always best!

At Iowa we now offer a CI fellowship for those interested after residency. As technology and AI continue to integrate more into medicine this is becoming extremely important! There are even options for combined fellowships! Message if interested!

Bilirubin is Bad for the Brain! Who knew? We have known for a very long time that bilirubin can cause an encephalopathy leading to acute and chronic clinical impacts, the most severe chronic impacts being kernicterus, a disabling movement …

Interesting discussion on Bilirubin and the debate on if antioxidant effect is helpful or bilirubin is harmful especially in the tiniest babies. At Iowa we choose the more aggressive approach of starting phototherapy at 5 for <1000g babies.
