1. INTRODUCTION
Breastfeeding infants and babies is lifesaving in emergencies. In emergencies, experiences such as displacement, disrupted services, gender-based violence (GBV), lack of privacy, donations of breast milk substitutes (BMS) including infant formula, and poor mental and physical wellbeing can create a situation in which breastfeeding is particularly challenging.
Mothers and caregivers who do not have access to adequate support to prevent and overcome breastfeeding problems are more likely to introduce supplements and stop breastfeeding early. As such, there is an increased need for alternative feeding options, such as wet nursing, to safeguard the health and nutrition of infants and young children when their own mothers are unable to breastfeed or provide sufficient breast milk.
2. OBJECTIVES OF THE SCREENING TOOL
This screening tool accompanies the UNICEF (2025) Technical and Operational Guidance on Supporting Access to Breast Milk Through Wet Nursing in Emergencies.

In June, @savethechildren.org @altusaid.altgov.info & the IFE Core Group developed a wet nurse screening tool for frontline health workers. It includes a medial history of the prospective wet nurse, and an assessment of her own child and coercion prevention.

Which is why the WHO/UNICEF standard for #IYCFE is that a board-certified lactation consultant or lactation physician evaluate each dyad in a disaster area. It IS about formula because it is a product that cost money and prep requires a clean surface, potable water, and heat for boiling.

This is why predatory formula marketing matters. This is why breastfeeding vs. formula is not a “lifestyle choice” but a *health equity issue.*

The more people in a community who are lactating, the more buffered the children in that community are from food insecurity.
