His daughter made an impromptu visit and found the ‘carers’ had
abandoned visits after day 1, and he was still sat in his soiled hospital pyjamas.

The main outcomes in #NOF improve with orthogeries input and timely ASAP surgery. Unnecessary delay is bad.
(I will confess I'm not great at lateral spinals and worry about positioning patients with #NOF with just a block and a bit of ketamine/fentanyl)

So a few years back, I saw a young reg putting in a 16g cannula without LA into an frail 80yo woman for a #NOF.
I took him aside and invited him to let me put a similar cannula into his arm without LA, which to his great credit he did.
I got it in first go; but he always used LA after that.
Just saying I wouldn't do it for a Saturday #NOF but I would do it in other specific situations.
I’m glad anesthetists are talking about the issues / concerns with positioning for a spinal in an elderly pt with #nof .
I give a GA with FIB . If a patient’s too unwell / unfit for a GA ( rare)then I have a discussion with family about futile surgery & palliation ( non operative ) .
I preferably wouldn’t do a spinal regardless of the clopidogrel for #NOF . These cases can go quick (45 minutes door to door) or end up as a 4 hour epos with cerclage and indecent blood loss. I don’t see the upside for the patient.

At the Royal Adelaide Hospital we have an Advanced Recovery Room Care ARRC unit. All #NOF go there and they can run metaraminol infusions overnight.
Started by Prof Guy Ludbrook, it is staffed by consultant anaesthetists and RMO’s.

In the context of #NoF in the very frail, the argument is that fixing the hip reduces pain, and by so doing you provide comfort.
Where the difference lies with an acute abdomen is that e.g. an ITU admission for multi-organ support is itself considered a distressing intervention.

Also remember the coroner is asking questions for family too. e.g. coroner may ask you to explain how someone "came in with a #NOF and left in a box with a death certificate saying pneumonia" - if that is what the family have been expressing. Helps families join the dots. n/2

We used to quote 10% mortality at a month and ⅓ at a year; might be old estimates though?
Certainly a high risk group, with a significant subset in whom fixation is a palliative intervention #NoF
Thanks for shining a light on this. We are trying to get a family member home after a #NOF and not into aged care. We can’t apply for an assessment for a home care package until they get home and if approved the wait to access the package is ~12 months . The aged care system is failing our elders.
The bestest boy enjoyed his first proper snow. We don't often get snow, I think because we are by the coast.
I turn into an 80 year old lady when it gets remotely slippy looking and shuffle along like I'm about to fall and end up with a #NOF 😂🙈
The national Hip Fracture Database shows the ave age for women is 84 and for men is 83 BJ (8/10 are women) with 7-10% dying within 30 days, roughly half related to the #NOF and the rest other reasons.
Is the pt with the #NOF generally older than the pt diagnosed with Ca lung? And therefore more likely to have other co-morbidities?
