One thing we’ve been doing in #PatientSafety and #SafetyScience (more in the literature than practice, still) has been moving away from a focus on “error” and toward a focus on harm and near-misses, with the recognition that most errors don’t cause harm, many harms aren’t caused by “error” per se…
