M proteins are coiled-coil fibrils covering #StrepA. Their N-terminal regions, presented away from the bacterial surface, are hypervariable. For the >200 M protein variants, these regions are generally predicted to form coiled coils or lack tertiary structure. However, M3 is different.
emm3 type #StrepA bacteria are highly prevalent, frequently causing severe streptococcal diseases such as invasive soft tissue infections #iGAS or rheumatic heart disease. Their dominant surface protein is M3 which, among other things, allows the bacteria to bind tightly to collagens.
About time to post about our recent paper on the interaction of the group A #streptococcus #StrepA M3 protein with collagen. Took over a decade to get here. I always thought it mad to embark on structural work with collagens. Or M proteins, for that matter. elifesciences.org/articles/105...
Great work from @nvansorge.bsky.social & colleagues on invasive Strep A. They found that giving preventative antibiotics to all household contacts cut secondary infection risk by more than four times. A simple policy change that could save lives.

What is more alarming is that those who die, sadly die quickly esp children ; blaming any single person for a strep A death is not sensible.

Children aged 3-4 were ~1 year behind in #StrepA immunity after the pandemic
This is the age group that experienced the greatest increase in life-threatening Strep A in 2022-23.
We saw substantial changes in detection of common pathogens like #StrepA during the pandemic as previously reported.
We measured #StrepA antibodies in samples from children recruited to the PERFORM and DIAMONDS studies before and after the pandemic.
We focused on the group who were as representative as possible of children in the wider population excluding those with definite bacterial and viral infections.

Many, perhaps most, of the deaths from ‘nephropathies’ (8%) and heart disease (12%) would have been infection-related so >2/3 deaths likely infection-related. Many, probably most, due to APSGN & RHD (i.e. #StrepA). Guess what? 🇺🇸 made a vaccine (it worked). 🌏 needs to do it again.

Really the answer to this degree of unpredictability (known unknowns) is a #StrepA vaccine not more data. If there’s a 🇺🇸 report soon of 👶🏼 with HiB facial cellulitis & mum with HiB mastitis, we won’t need more data to know who to blame.
