【完全保存版】難聴児の聴力検査まとめ｜年齢別に見る検査の流れとポイント解説！ 聴力検査って種類がたくさんあってよくわからない… BOA、COR、ABR、ASSR、純音聴力検査、語音明瞭度検査……。病院で説明を受けたはずなのに、あとから結果用紙を見返すと、よくわからない専門用語だけが並んでいるように感じることはありませんか？私自身、子どもが小さい頃は「この検査で何がわかっているんだろう」「次はなぜ別の検査をするんだろう」と、そのつながりが見えないまま不安だけが残っていました。しかし、振り返ってみると、聴力検査はバラバラに存在しているのではなく、子どもの成長に合わせて「わかることが少しずつ増えていく仕組み」だったのです。この記事では、0歳から小学生までの間に行われる聴力検査を年齢の流れに沿って整理しました。全体像をひと目でつかめるようにまとめています。お子さんの年齢を思い浮かべながら「うちは今このあたりかな」という気持ちで読み進めてください。この記事でわかること今の年齢でどんな検査が行われやすいのかその検査でどんなことがわかるのか次の時期には何が見えてくるのか今の年齢で、今のわが子にとって、どんな情報が得られていれば十分なのか。この記事が、聴力検査を「不安のかたまり」ではなく、わが子の聞こえ方を知るための道しるべとして整理する手助けになればうれしいです。ここからは、0歳から小学生ごろまでの流れに沿って、年齢ごとに行われやすい聴力検査と、その時期に「どんなことがわかるのか」を見ていきます。検査名を覚えることよりも、今の年齢で何が確認できていれば安心なのか、その目安として読んでください。新生児～6ヶ月｜眠っている間に「聞こえの土台」を確認する時期生まれてすぐの赤ちゃんは「聞こえたよ」と教えてくれることができません。そのため、この時期の聴力検査では、赤ちゃん自身の反応や脳の動きを手がかりにして聞こえの状態を確認していきます。 0歳ごろに行われることが多いのは、音に対する脳の反応を見る検査です。眠っている間に行うことが多く、赤ちゃんに何かをがんばってもらう検査ではありません。新生児聴覚スクリーニング検査（聞こえにくさの可能性がないかのチェック）新生児聴覚スクリーニング検査は、生まれてすぐの赤ちゃんを対象に行われる検査です。この検査の目的は「聞こえにくさの可能性がないか」をできるだけ早い段階で見つけることです。短時間で行えるのが特徴で、赤ちゃんが眠っている間に実施されることも多く、体への負担はほとんどありません。ただし、この検査は聞こえ方を詳しく調べるためのものではなく、あくまで「ふるい分け」の役割を持つ検査です。そのため、結果によっては「もう少し詳しい検査をしてみましょう」と次の検査につながることがあります。赤ちゃんの聞こえを「リファー：聞こえにくい可能性がある」「パス：現段階では聞こえている」の２つにふるい分けることができるこの検査の医学的な仕組み新生児聴覚スクリーニング検査では、主に2つの方法が使われます。 OAE(耳音響放射) 耳の奥にある内耳の有毛細胞が正常に機能しているかを確認します。音を聞かせたときに内耳から返ってくる微弱な音を測定する仕組みです。自動ABR(自動聴性脳幹反応) 音が脳まで届いているかを簡易的に確認します。どちらも客観的な測定が可能で、赤ちゃんが眠っている間に数分で完了します。新生児聴覚スクリーニング検査のなぜ？どうして？ Q 検査結果の信頼性は？この検査の感度(聴覚障害を見逃さない確率)は非常に高く設定されていますが、一方で偽陽性(本当は問題がないのにリファーと判定される)も一定数発生します。リファー(要再検査)と判定されても、実際に聴覚障害があるのは約10人に1人程度です。耳垢や羊水の残り、検査時の体動などで一時的に反応が出にくいこともあります。そのため、リファーと判定されても過度に心配せず、精密検査を受けることが大切です。 Q なぜこの時期に行うの？先天性聴覚障害の発生頻度は新生児1,000人に1〜2人とされており、比較的頻度の高い先天性疾患です。聴覚は言語発達や認知発達に深く関わるため、生後6ヶ月までに発見し、適切な支援を開始することで、言語発達への影響を最小限に抑えられることが分かっているからです。日本耳鼻咽喉科学会も「生後3日以内のスクリーニング、生後1ヶ月までの精密検査、生後6ヶ月までの療育開始」を推奨しています。 Q この検査でわかったことは、その後どう役立つの? 新生児聴覚スクリーニング検査は、聴覚障害の早期発見システムの入り口として位置づけられています。この検査でリファーとなった場合、精密検査(ABRやASSR)へと進み、診断が確定すれば補聴器装用や療育支援へとつながっていきます。現在、日本では出生児の約90%以上がこの検査を受けており、公費助成を行う自治体も増えています。新生児聴覚スクリーニング検査については、こちらの記事で詳しく解説しています。 ABR検査：聴性脳幹反応検査（音に対する脳の反応を測る検査） ABR検査は、音に対する脳の反応を測る検査です。新生児聴覚スクリーニングでリファーとなった赤ちゃんへの精密検査として、また難聴の確定診断に使用されます。音を聞いたときに、耳から脳へと信号が伝わっているかを脳波として記録することで、聞こえの状態を確認します。赤ちゃん自身が反応を示す必要がないため、眠っている間に行われることが多い検査です。診断や補聴器・支援を考えるための大切な情報になる検査のひとつです。どのくらいの大きさの音で反応が出るかをもとに、おおまかな聴力レベルや左右差を推測することができる。この検査の医学的な仕組み ABR検査では、耳にイヤホンをつけて音(主にクリック音やトーンバースト音)を聞かせ、その際に発生する脳幹の電気活動を頭皮上の電極で記録します。音が耳に入ってから脳幹を通過するまでの間に、I波からV波までの5つの波形が現れます。特にV波は最も安定して観察でき、このV波が何デシベル(dB)の音で出現するかを「V波閾値」として測定します。 V波閾値は実際の聴力レベルに近い値を示すため、診断の重要な指標となります。検査結果の信頼性 Q…
