Nebular Cortical Cataract in Stickler Syndrome A 12-year-old boy with a family history of Stickler syndrome presented with blurry vision. Examination revealed an empty-appearing vitreous and bilateral curvilinear lenticular opacities, consistent with cortical cataracts (right eye; A, right gaze; B, retro-illumination). Genetic testing confirmed a pathologic COL2A1 variant. Stickler syndrome is an autosomal dominant collagen disorder with well-known ophthalmic manifestations including severe myopia and lattice degeneration. There is a high lifetime risk of retinal detachment, which this patient developed years later.
