Clinical Simulation Professional Development: A Scaffold Approach Healthcare is a dynamic field with ever-evolving complexities, demanding simulation-based professionals to stay abreast of technological advancements, evolving practices, and enhanced understanding for optimal patient-centric care. Evaluation and Measurement of Clinical Simulation-Based Experiences Jill Sanko is an award winning simulation researcher with over 20 years experience in the healthcare simulation space. She began her career in simulation-based education at the National Institutes of Health (NIH) Clinical Center as the founding Associate Director of the Simulation and Patient Safety Program. Moving back to Miami 2008 for the next 12 years she worked in several research and teaching positions for U of Miami and Jackson Memorial Hospital further establishing herself as a leading expert in healthcare simulation to improve healthcare safety. Dr. Sanko holds several certifications across related areas including, board certification as an Acute Care NP, as an Advanced Certified Simulation Educator, a Friday Night at the ER Team Learning Simulation Leader, and a Fellow of the Society for Simulation Academy. Her research interests include the intersection of simulation-based inter-professional team education to improve patient safety and the incorporation of systems thinking and systems-based practice in healthcare settings and education. She has over 40 articles, book chapters and other publications and has presented her work nationally and internationally. Her research efforts have also garnered awards from the Society for Simulation in Healthcare for her work Exploring the Relationship Between Team Cohesion and Team Performance, Simulation to Improve Situational Awareness, Simulation to Test a Hand Hygiene Reminding System, Simulation as a Means to Improve Adverse Event Reporting, and Exploration of Systems Thinking on Patterns of Adverse Event Reporting. She is active in her community of practice as well; currently serving in several roles within the Society for Simulation in Healthcare including as co-chair of the distance simulation affinity group, chair of the meetings oversight committee, and chair of the Academy of Simulation Fellows. She is also part of the executive team for the Distance Simulation Collaboration.

Role of Clinical Simulation Operation Specialists in Houssaye’s Educational Triangle For just over 20 years, Guy Stotzer has had the chance to develop his skills in an environment that is conducive to innovative developments at La Source School of Nursing. Simulation in Healthcare has been and still is one of these developments. From 2010 to 2011, Guy set up technologies for a Clinical Practice Laboratory dedicated to Simulation in Healthcare (720m2 / 7,750ft2). From 2015 to 2018, he pursued his efforts and his management’s wish to set up a technological structure covering 6,000m2 (64,583ft2), including a 2,500m2 (26,910 ft2) Simulated Hospital. Guy has managed these two centers as Technical Manager. Today he is implementing other educational technologies in the field of Healthcare Simulation. After spending 2 years in the Faculty of law and 3 years in the Faculty of Arts at Swiss Universities, Guy obtained a Graduate Diploma, Arts and Sciences of Broadcasting. He also obtained an ICT Manager Federal Diploma. He got the CHSOS-A in 2020. Guy trained in simulation with Dr. Pamela R. Jeffries and her team (USA) and with Master mentor Suppiah Nagammal and her team (Singapore). He is member of the Society for Simulation in Healthcare (SSH) and involved in the Technical Committee of the Société Francophone de Simulation en Santé (SoFraSimS), France.

Enhancing New Graduate Nurse Performance in Rapid Response Teams with Nursing Simulation There is substantial literature about New Graduate Nurses (NGN’s) perceptions of readiness for code blue management or their perceived comfort level in recognizing and responding to a change in patient condition. However, there is minimal quantitative data available on the outcomes for simulation-based training for code blue responses. There is a paucity in literature describing interventions that incorporate assessment such as Objective Structured Clinical Exams (OSCE). The purpose of the study is to address a gap in knowledge concerning new graduate nurses’ perceptions of their comfort and confidence levels when participating in a rapid response or code. The study will also provide quantitative and qualitative data on the effect of simulation on the NGN’s ability to recognize, respond to and deploy a rapid response or code. The quasi-experimental mixed methods study will employ the use of a standardized high-fidelity simulation activity and Objective Structured Clinical Exam (OSCE) with a pre-/posttest. Data analysis will consist of quantitative statistical analysis and Naturalistic Inquiry methods to obtain qualitative data. Guided interviews with semi-structured, open-ended questions will be utilized. Participant responses will be recorded and transcribed. The team will be scored as a group for the simulation performance while individual competence will be assessed via a brief questionnaire at the end of the simulation. The OSCE will be evaluated according to a rubric/checklist utilizing binary scoring of pass/fail and participant score on resuscitation quality utilizing video capture and simulation software. The theoretical framework of interest is Erickson’s Role Modeling and Benner’s Novice to Expert theory. The debriefing model used was the Advocacy-Inquiry Method to encourage self-reflection of learner actions. The justification for the selected study design is that the pre-/posttest with an intervention can detect effect or causality. The population of study is new graduate nurses currently in orientation or residency programs. Results 80 students participated in the study. There was a statistically significant difference between pre-/post scores (p-value <0.001). There was an improved recognition of patient decline among new graduate nurses post nursing simulation activity. Evaluate resuscitation quality in new graduate nurses using nursing simulation. Integrate and apply organizational policy and procedures during simulated nursing activities. Describe best practices through academic discourse among healthcare simulationists.

What I Wish My Faculty Knew: Learner Perspectives on Clinical Simulation About: This CE presentation will present learner perspectives on healthcare simulation education with suggestions on how to help students become practice-ready. While clinical faculty may have a strong connection with their clinical students and a valid perception of the learner experience of clinical simulation, the professional roles of educators and researchers inherently limit what students share. This resistance to providing honest learner feedback temporally and spatially limits the faculty’s understanding of students’ experiences to the educational or research setting. This CE presentation aims to better inform medical simulation educators and researchers about student engagement and how to construct better learning experiences and support students in simulated training. Students of varying experience levels will present their perspectives and experiences with healthcare simulation and how to support their learning in healthcare simulation to better become “practice-ready”. Describe common clinical student perceptions of healthcare simulation-based education. Identify insights from student perception that can be applied to clinical simulation education. Compare and contrast student perceptions with existing faculty beliefs about student experiences to help learners become practice-ready.

Replacing Traditional Clinical Rotations with Nursing Simulation A groundbreaking study done in the USA provided empirical evidence that substituting high-quality simulation-based experiences for up to 50% of traditional clinical hours was just as effective in providing learners with requisite knowledge and skills students would need upon graduation (Hayden, Keegan, Kardong-Edgren, & Smiley, 2014). In simulated based learning, learners work on complex problems in controlled environments that mimic real word scenarios (Lateef, 2010). Simulation-based education (SBE) is used in healthcare to facilitate students’ acquisition knowledge and skills through working with ‘ex-vivo’ patients in controlled safe learning environments without compromising the safety of real patients (Alinier & Oriot, 2022; Alinier & Platt, 2014; Aqel & Ahmad, 2014; Shin, Park & Kim, 2015). Our project replaced 25% of clinical hours in medicine and surgery with simulations in a practical nursing program. A total of 22 students and their 3 instructors participated in this project. Evidence based approach was used throughout the design and implementation of the project to make sure that learning outcomes were met without compromising the quality of students’ learning. We documented the process of instructor training, curriculum development, implementation and feedback from students, staff and instructors. The project’s main purpose was to explore ways of addressing three important issues: The modalities of simulation included medium to high-fidelity manikins (16%), standardized patients (SPs) (6%) and virtual reality (3%). EDI learning opportunities were woven into relevant scenarios. Students and instructor feedback shows that learners gained confidence, practiced skills they would not have encountered in traditional clinical, learnt the importance of critical reflective practice, collaboration, problem solving skills and resolving issues related to equity diversity and inclusion. The learnings from this project are being used to scale up the simulated based learning by replacing 50% of traditional clinical hours in continuing care for the internationally trained nurses that is currently running. The following academic year we will take these learnings and scale to over 400 students per semester at 25% replacement. Our presentation will focus on the disruption of traditional clinical students’ experiences and the relearning process of new pedagogical strategies in nurse education. We will outline the pre-implementation activities, implementation process, the outcomes of the project and what we learned as discussion the way forward. Understand how replacing clinical education rotation hours with nursing simulation as an approach can enhance clinical learning outcomes. Understand what steps were taken, logistics, and operational resources were used to execute replacing clinical hours with healthcare simulation. Apply these learnings from the presentation to the participant’s organization, to initiate or build on replacing clinical rotation hours with clinical simulation activities.

Transforming Nursing Education: Embracing Virtual Reality in Training Virtual Reality (VR) for healthcare professionals’ education is emerging as a viable and cost-effective alternative to traditional manikin-based simulations. Despite the proven effectiveness of simulation-based education, many hospitals face significant challenges, such as limited space, funding, and staffing, which hinder the implementation of small-group learning. Chen (2020) highlights that VR is becoming increasingly affordable, making it an attractive solution for enhancing nursing education. Join us to explore how VR is transforming the landscape of professional training in healthcare. Purpose: This pilot study aims to explore nurses’ acceptance and use of virtual reality (VR) technologies in healthcare education. The aim was to examine the applicability of VR technologies in healthcare education. Framework: This study is a useability pilot that includes a quantitative questionnaire from a cohort of nurse participants who voluntarily observe an immersive VR simulation. Method: Survey materials based on the Unified Theory of Acceptance and Use of Technology (UTAUT). A post-evaluation, inclusive of a Likert Scale and open-ended questions, was used to explore nurses’ acceptance of VR technology in professional nursing education. The data were analyzed using descriptive statistics and thematic analysis. Describe the concept of Virtual Reality (VR) and its application within nursing education and service. Discuss the integration of VR within the contexts of Andragogy Principles and Learning Theory. Assess the role and effectiveness of VR in enhancing nursing education and service.

Location! Location! Location! Comparing Simulation Debriefing Spaces In this CE webinar will focus on the critical consideration of the location of the simulation debrief. Reviewing the paper “Location! Location! Location! Comparing Simulation Debriefing Spaces” the webinar aims to have a discussion on the benefits, practicality and learning experiences of simulation location debriefing. A review of the background leading to this study, methodology and results will be described. The University of Manitoba, College of Nursing, has a comprehensive multi-modal simulation program. After undergoing a nursing seat expansion in Fall 2023 (annual admission of 360 students), the simulation team was looking for solutions to create space and still hold high quality, optimal simulations. The study was created to assess students’ debriefing experiences and the importance of the debriefing experience in a one-room debriefing space as compared to a traditional multiroom debriefing space. This webinar aims to encourage educators and administrators to think outside of the traditional norms for debriefing locations. Participants will gain an understanding of how the debriefing location can impact the effectiveness of simulation-based learning. Describe the different locations of healthcare simulation debriefing. Understand the unique challenges and opportunities of debriefing locations in healthcare simulation and the impact they can have on clinical training outcomes. Envision future areas of research around debriefing locations in clinical education and training for better education and patient care outcomes.

Maximizing High Fidelity Trauma Simulators in MASCAL Exercises Operations-based Mass Casualty (MASCAL) trauma simulation exercises are a common occurrence in EMS and military training. Frequently complex training operations, these MCI training exercises usually rely on role players with minimal integration of high-fidelity manikins. The reasons are many. Moulaged role players are realistic and flexible with fluid situations. Beyond that, most high-fidelity manikins are too fragile to leave the clinical training room and too complicated to operate on the fly. A drawback of role players is the challenge of performing lifesaving procedures realistically and objectively measuring care through the treatment and evacuation chain. This seminar will share MASCAL planning resources and discuss micro simulations within MASCAL events designed to truly test and measure participant performance with high fidelity human patient simulators. Learners will be introduced to the SORT methodology for planning simulations, a rapid, simple process to develop simulation pedagogy quickly and effectively Analyze MASCAL learning objectives at the macro and micro level and identify learning opportunities through medical simulation. Learn the SORT method to develop trauma simulations. Apply SORT within a larger MASCAL Scenario to create a micro-simulation against defined objectives.

The Swansea Univerity SUSIM project has been a cross-site, cross-faculty major investment, and commitment to expanding simulation-based educational methodologies for undergraduate and postgraduate learners. The programme is also working with local Health Boards, Government bodies, communities, and industry as part of this strategy. The session will cover the key factors to consider when launching or taking part in simulation expansion projects of any scale. The discussion will draw on this current SIM project and programme as well as previous international project experiences and will explore different perspectives such as faculty, partnerships, and considerations relating to innovation and sustainability. Share the journey of a medical simulation and immersive learning expansion project at a major UK university. Discuss key considerations of each healthcare simulation project phase from design, commissioning, opening, and sustainability. Describe faculty growth and professional development of clinical simulation team members during each of the phases. Jo has worked in clinical, education and senior leadership roles in the UK, Middle East & Australia throughout her 30 yr career. Jo has worked in simulation roles for Queensland Health, co-designed, directed & commissioned the Sidra Medical Simulation Center and programmes, leading teams into operations, and supporting the opening of a 450-bedded women and children’s hospital using simulation for mass systems testing. She co-founded the Qatar Simulation Consortium and MENA simulation programmes and is an advocate of immersive learning methodologies and faculty development in simulation-based learning, effective debriefing and team performance in healthcare systems. Jo started her new position with Swansea University in July 2021 establishing the SUSIM programmes and Chairs the Interprofessional Simulation Education Working Group. Jo is directing the expansion of SBE methodologies, research, and collaborations, including the design and recent opening of the Swansea University Simulation and Immersive Learning Centre and a linked second site in Carmarthenshire in Wales. These facilities house the largest current installation of immersive wall technology globally for healthcare training. Jo also oversees numerous simulation research projects and is currently the Principal Investigator on a Higher Education Funding Council of Wales (HEFCW) Virtual Reality Project which was awarded £900,000 of funding towards the creation of clinical VR content in English and Welsh languages.

Reflective Healthcare Simulation Feedback Techniques for Standardized Patients Feedback is arguably the most impactful, key component of the work Simulated Patients aka Standardized Patients (SPs) contribute to healthcare simulation. How do we ensure we are meeting the standards of best practice regarding feedback? Research on successful SP feedback training techniques is limited, and the adoption of active methodologies is still low. We have chosen to transition away from feedback structures, such as the popular “feedback sandwich,” which Simulated Participants reported finding limiting and less authentic. The Reflective Feedback Technique (RFT) was created by modifications to Wiggins’ Seven Keys to Effective Feedback to suit the needs of simulation-based education. A process for continuous training improvements for SP’s established quality and efficacy in the verbal feedback components, followed by an ongoing training strategy focused on self-reflection that references John Hattie’s Visible Learning. These ideals also informed the creation of a post-encounter feedback reflection survey for quality assurance. A survey of 60+ SPs revealed all felt more confident and comfortable to give verbal feedback to learners under these new guidelines. SPs reported success with transparent, collaborative, learner-led feedback sessions. Additionally, results revealed the faculty’s learning objectives and more directly met the individual needs of each learner. Faculty tracked and provided continual improvement in the quality of these feedback sessions with post-encounter reflection surveys. This new feedback technique led to greater educational outcomes for learners and more effective feedback discussions for SPs. Many of our SPs work at multiple institutions and report increased certainty in impactful feedback sessions under this format. Moreover, SPs believe they facilitated these sessions with greater confidence and created more positive experiences through this reflection method. State how to provide more individualized SP feedback sessions for clinical learners.

Using the SimVS time-stamped debrief function, the presenters will review debriefing and record-keeping as well. The presenters will show how instructors can simply incorporate three elements of the interconnected SimVS ecosystem: their patient monitor, the nurse call system and the IV pump simulator to create an impactful experience. Identify the pre-planning and logistics components necessary for successful medical simulation scenario operations. Show how to develop communication objectives in clinical simulation scenarios. Learn how to design clinical simulation scenarios for high realism. Greg Vis developed the SimVS line of medical simulation devices beginning in 2017. Greg has been involved in healthcare education for over 20 years. He caught the simulation bug filming a series of skills videos using the brand new Noelle manikin in 2002 and has never looked back. The potential of this new learning method to improve healthcare and accelerate student learning was captivating back then and continues today. SimVS has developed a number of innovative first in simulation including the world’s first tablet based mechanical ventilator in 2018 and continuing that tradition in 2023 with the forthcoming release of SimVS-IV, the world’s first IV pump simulation. Overcoming Obstacles to Trauma Simulation Trauma is a leading cause of death in people under the age of 45 and fourth leading cause of death overall for all ages.1 The American College of Surgeons has created several course geared to advance trauma practice not only in the U.S. but globally.2 In this CE/CME webinar, Dr Luis Llerena will discuss valuable simulation methodology used to enhance trauma training while adhering to adult learning theory. Malcolm Knowles developed the concept of Adult Learning Theory in 1973.3 This theory holds that adults learn by having engagement and respect during training. Specific trauma courses presented at one major university simulation center will serve as the example. The specific trauma courses discussed will be the following: Advanced Trauma Life Support (ATLS), Advanced Trauma Operative Management (ATOM), Advanced Surgical Skills for Exposure in Trauma (ASSET), Trauma Team training, In situ training, and Disaster course. These courses faced significant obstacles to learner groups, instructors, and deployment. This webinar will describe solutions that were implanted. Hospital at Home: Development of New Care Model for Medical Simulation About: This CE webinar will host an interprofessional panel who will discuss their collaborative effort to use medical simulation to validate workflows and identify latent safety threats before implementing a new Hospital at Home program that provides acute hospital care in a patient’s home. The interdisciplinary panel of speakers will discuss their experience of using system-focused simulation to test the quality and safety of a new and complex clinical care model, Hospital at Home, in the pre-implementation phase. Hospital at Home is an innovative care model providing hospital-level care in the patient’s home, which allows eligible patients to receive acute care and recover in the comfort of their homes. It entails many of the same care processes occurring in the brick and mortal hospital, including clinician visits, vital signs, lab and diagnostic studies, medications, auxiliary therapies, and meals. In essence, this entailed developing a new hospital system outside the hospital, inclusive of the workflows, communication patterns, information technology infrastructure and care processes. A large interdisciplinary effort was undertaken to delineate the processes and best workflows from entry into the program through discharge. Kaizen events, steering committee meetings, and patient-centered focus groups drove the creation of the standard work. Upon creation of the standard work, several tabletop simulations were conducted to confirm the appropriate processes and personnel, and identify potential system stress-points. This was then followed by “Day in The Life” (DiTL) simulations that included realistic scenarios with simulated patients and were designed to (1) validate and test – equipment, communication, procedures, and workflows and (2) discover – latent safety threats, resolutions to issues, opportunities to optimize quality of care and patient safety. Simulations encompassed the Hospital at Home process by beginning with patient eligibility and consent, the hospital stay, transitioning to home, care at home as a Hospital at Home patient and lastly discharge. Each simulation had participants in-person and via Zoom with members from the full interprofessional team (e.g., nursing, physicians, pharmacists, social workers, home nursing) and the hospital system team (e.g., IT, supply chain, pharmacy, operational excellence, simulation). Throughout the phases of the Hospital at Home program, the teams participated in focused interval debriefs centered on the SEIPS 2.0 (Systems Engineering Initiative for Patient Safety) human factors framework. The debriefs culminated in approximately 120 opportunities for improvement that were then triaged by criticality and importance to be addressed longitudinally. Following the simulated sessions, the clinical team was equipped with valuable insights that significantly improved readiness to enroll the first patient in the Hospital at Home program. The team analyzed the outcomes, feedback, and learning points from the simulations and promptly integrated them into protocols and procedures. This involved refining the patient enrollment approach, fine-tuning equipment use, honing emergency response protocols, and enhancing interpersonal communication strategies to ensure a patient-centric approach. By learning from the simulated experiences, the team created a more robust and effective system, significantly reducing the potential for errors or mishaps. As a result, the team members were more confident in their ability to provide acute hospital-level care at home and better prepared to meet the unique needs and preferences of the first patient in the program. This iterative learning process ultimately elevated the standard of care provided, optimizing the patient experience from the get-go. Characterize the integration of simulation into a multidisciplinary effort for the development of a new clinical care model. Describe the collaborative use of simulation and operational excellence programs in the pre-implementation testing of the new Hospital at Home program. Transforming VR Clinical Education with the Cognitive Affective Model in Immersive Learning The Cognitive Affective Model of Immersive Learning (CAMIL) provides a theoretical framework to understand and optimize learning within immersive virtual reality (IVR) environments. CAMIL clarifies how learning occurs in IVR by highlighting the psychological affordances of presence and agency. CAMIL identifies six key affective and cognitive factors influencing IVR learning: interest, motivation, self-efficacy, embodiment, cognitive load, and self-regulation. The presenters will discuss how professionals can develop a holistic understanding of competency, integrating diverse lenses to enhance interprofessional practice and improve patient outcomes. Accurate competency assessment is vital for healthcare professionals to provide safe patient care. The panel will review Objective Structured Clinical Examinations (OSCEs) for standardized evaluations (Lioce et al., 2020) and discuss various assessment modalities, including summative, formative, and high-stakes assessments. Successful Integration of Virtual Reality into Healthcare Facilities Competency training, assessment and demonstration are required components in ensuring continuous quality nursing care in healthcare facilities. Healthcare Simulation has long been the standard for the demonstration of learned skills and algorithm application. Traditionally, this has included high- and low- fidelity manikin learning labs and standardized patient scenario exercises. The expense and time requirements of these methods can be a barrier to usage, so many facilities are beginning to use virtual reality as a training modality for competency training and assessment. Virtual reality (and other mixed reality modalities) requires the use of a headset and hand interface tools (in most cases controllers). The use of headsets may not be intuitive for some non-digital populations of learners and there may be some resistance to using these. This resistance can be tempered or even avoided by pre-emptive planning. This CE webinar will discuss methods to successfully introduce VR to a non-digital population of nurses in a healthcare facility. Women make up approximately 80% of all healthcare workers, and the average age of RNs is 46 years old. Both demographics are underrepresented in identified virtual reality user statistics, where only 16% of women had ever tried virtual reality and 50% of surveyed women stated that they had no interest in the technology. Age-wise, only 18% of regular VR users were over 45 and over 70% of these were male. Additionally, studies show that women are more likely to experience (and anticipate!) virtual reality sickness This can present a challenge for nurse educators introducing this technology as a staff competency training and assessment tool. Pre-emptive assessment and training can mitigate or prevent a reluctance or refusal to use VR. This presentation discusses pre-training staff assessment, training communication techniques, orientation and room preparation, comfort measures and methods to measure competency when in-headset training is not achievable. A lack of preparation and messaging can result in staff refusing to use the headsets and potentially shuttering the platform. Virtual reality has many benefits for nursing competency training and assessment for healthcare facilities when introduced with thought and careful consideration. Identify characteristics of nursing staff in regards to VR technology and usage. Institute proactive measures to promote successful usage of VR in the nurse population. Develop alternative methods of competency assessment using the VR platform outside of the headset.
