Triaging Casual From Critical—Leveraging Machine Learning to Detect Self-Harm and Suicide Risks for Youth on #SocialMedia #SoMe #hcsm: Algorithm Development and Validation Study Background: This study aims to detect self-harm or suicide (SH-S) ideation language used by youth (aged 13-21 y) in their private Instagram (Meta) conversations. While automated #MentalHealth tools have shown promise, there remains a gap in understanding how nuanced youth language around SH-S can be effectively identified. Objective: Our work aimed to develop interpretable models that go beyond binary classification to recognize the spectrum of SH-S expressions. Methods: We analyzed a dataset of Instagram private conversations donated by youth. A range of traditional machine learning models (support vector machine, random forest, Naive Bayes, and extreme gradient boosting) and transformer-based architectures (Bidirectional Encoder Representations from Transformers and Distilled Bidirectional Encoder Representations from Transformers) were trained and evaluated. In addition to raw text, we incorporated contextual, psycholinguistic (linguistic injury word count), sentiment (Valence Aware Dictionary and Sentiment Reasoner), and lexical (term frequency–inverse document frequency) features to improve detection accuracy. We further explored how increasing conversational context—from message-level to subconversation level—affected model performance. Results: Distilled Bidirectional Encoder Representations from Transformers demonstrated a good performance in identifying the presence of SH-S behaviors within individual messages, achieving an accuracy of 99%. However, when tasked with a more fine-grained classification—differentiating among “self” (personal accounts of SH-S), “other” (references to SH-S experiences involving others), and “hyperbole” (sarcastic, humorous, or exaggerated mentions not indicative of genuine risk)—the model’s accuracy declined to 89%. Notably, by expanding the input window to include a broader conversational context, the model’s performance on these granular categories improved to 91%, highlighting the importance of contextual understanding when distinguishing between subtle variations in SH-S discourse. Conclusions: Our findings underscore the importance of designing SH-S automatic detection systems sensitive to the dynamic language of youth and #SocialMedia #SoMe #hcsm. Contextual and sentiment-aware models improve detection and provide a nuanced understanding of SH-S risk expression. This research lays the foundation for developing inclusive and ethically grounded interventions, while also calling for future work to validate these models across platforms and populations.

The Stanford Brainstorm #SocialMedia #SoMe #hcsm Safety Plan (SMS): Introducing a New Tool We propose the Stanford #SocialMedia #SoMe #hcsm Safety Plan as a user-friendly, collaborative, and effective tool to mitigate the imminent dangers and risks to #MentalHealth that are associated with #SocialMedia #SoMe #hcsm use by children, adolescents, and young adults. This tool is informed and inspired by suicide safety plans as part of suicide safety planning (SSP), which have long shaped the standard of care for psychiatric discharges from inpatient units, emergency rooms, and comprehensive psychiatric emergency programs (CPEPs), as well as longitudinal outpatient care following occurrences of suicidal ideation or suicide attempts. In many systems including those of the Veterans #Health Administration (VHA) where we first encountered them early in the course of our clinical training, they constitute an absolute requirement prior to discharge of the patient. Our #SocialMedia #SoMe #hcsm safety plan is to be used proactively, in times of normalcy as well as crisis. While parental controls for #Digital devices and online platforms, official legal age requirements for online accounts, and individual parenting #Approaches abound, there is a dearth of practical tools that youth, families, schools, and communities can use to shape and alter #SocialMedia #SoMe #hcsm use parameters, rules, and habits. Furthermore, providers in psychiatry, child and adolescent psychiatry, and #MentalHealth at large are often confronted with behaviors and issues related to #SocialMedia #SoMe #hcsm use during already time and resource-limited #Appointments, providing a massive opportunity for interventions that are harm reduction-oriented and easy to disseminate. While it has not yet been studied in a clinical trial, we have piloted its use with patients and families, worked on revisions and potential distinct versions of it, and presented it to larger audiences at #MentalHealth and technology conferences over the past year. The responses and feedback we have received, as well as reported anecdotal experiences with using it, have been overwhelmingly positive. An already unfolding child and adolescent #MentalHealth epidemic in the US has been aggravated and deepened partly by way of easy access to #SocialMedia #SoMe #hcsm (and #Digital-screen time) with inadequate safeguards and monitoring in place. #SocialMedia #SoMe #hcsm’s impacts and related interventions require a multitiered biopsychosocial and cultural #Approach: at the level of the individual child, the family, the school, the state, the market, and the nation. At the level of youth and their parents or caregivers, practical tools are desperately needed. We propose the Stanford Brainstorm #SocialMedia #SoMe #hcsm Safety Plan as one such significant tool.

Dashboard Intervention for Tracking #Digital #SocialMedia #SoMe #hcsm Activity in the Clinical Care of Individuals With Mood and #anxiety Disorders: Randomized Trial Background: #Digital social activity, defined as interactions on #SocialMedia #SoMe #hcsm and electronic communication platforms, has become increasingly important. Social factors impact #MentalHealth and can contribute to #depression and #anxiety. Therefore, incorporating #Digital social activity into routine #MentalHealth care has the potential to improve outcomes. Objective: To compare treatment augmented with an electronic dashboard of patient's #Digital social activity versus treatment-as-usual on patient-rated outcomes symptoms of #depression in a randomized trial of patients with mood and #anxiety disorders. Methods: We developed a personalized electronic dashboard summarizing a participant’s #Digital social activity. This dashboard, collaboratively discussed during #MentalHealth visits, was used to augment clinical care and tested in a randomized trial against treatment-as-usual. Clinicians and patients were recruited from outpatient psychiatry clinics. Patients were eligible if 12 years or older and receiving treatment for a mood or #anxiety disorder. Psychiatric symptoms measures for #depression (primary outcome measure) and #anxiety (secondary outcome measure) were obtained at each clinic visit as part of measurement-based standard of care. Baseline and 3-month follow up assessments included a measure of #MentalHealth status and therapeutic alliance measure. Also collected at each visit was a collateral information and clinical action scale. Results: A total of 103 patients were consented, 97 of whom were randomized to the dashboard intervention arm (n= 49) or treatment as usual (n= 48). There were no differences in psychiatry symptom rating scores or #MentalHealth status between the two arms. However, there was a significant increase in discussion of #Digital social activity with the intervention and it did not #Appear to change patient therapeutic alliance. Conclusions: Incorporation of a personalized electronic dashboard into clinical care was feasible and led to increased discussion of #Digital social activity, but there was no impact on #MentalHealth outcomes. Clinical Trial: The study is registered on ClinicalTrials.gov (NCT03925038).
